Grant Application Form
  To our Minnesota troops, if you have served in a combat zone since September of 2001, we'd like to share with you this modest grant as a thank you from the citizens of the State of Minnesota. To the families of our fallen Minnesota troops, a modest grant also is available for your family as a show of appreciation. To confirm your eligibility, please review the following Eligibility Details Document.

To apply for a grant, you have two options.
1) Enter Online. Please fill out the following online form. Once you have submitted it, you will be prompted to print, sign, attach documents and mail the form.     Online Grant Application Form                  
 
2) Enter Offline. If you would prefer not to enter through the online form, you can print this out and mail it in.
